What's included with my membership?

Your Root & Rise Family Health membership includes unlimited primary care office visits, longer appointment times, secure direct messaging, and personalized care focused on your whole health.

Together, we'll have the time to discuss nutrition, exercise, sleep, stress, relationships, preventive care, and lifestyle factors alongside traditional medical care to create a plan that's tailored to you.

Members also enjoy exclusive wellness benefits, including:

• 20% off IV vitamin infusions

• 20% off wellness injections

• One complimentary IV add-on with every IV infusion (up to a $20 value)

• One complimentary in-home visit upgrade each year

• One complimentary Mama Hydrate IV per pregnancy when administered during a qualifying postpartum visit

• Included office visits to review functional laboratory testing and adjust your treatment plan

Laboratory testing, imaging, medications, procedures, home visits, and services provided by outside facilities are billed separately unless otherwise noted.

No. Membership is not health insurance and does not replace insurance coverage.

We recommend maintaining health insurance to help cover things like specialist visits, hospitalizations, imaging, surgeries, emergency care, and other unexpected medical expenses.

Can I still use my insurance?

Absolutely.

Your membership provides direct access to your primary care provider and covers your membership benefits at Root & Rise Family Health.

You may still use your health insurance for laboratory testing, imaging, prescriptions, specialist visits, hospital care, and other covered healthcare services.

We are currently in the process of becoming in-network with major insurance plans. Once credentialing is complete, patients who choose to use their insurance for eligible office visits will be responsible for any applicable copays, coinsurance, deductibles, or other cost-sharing required by their insurance plan. Membership and insurance can work together, allowing you to enjoy the enhanced access and exclusive benefits of membership while using your insurance for covered services when available.

Yes.

Membership requires a three-month minimum commitment. After that, you may cancel anytime before your next billing cycle. Annual memberships are paid in full and renew each year unless canceled before renewal.

Are there any additional costs?

Your membership covers the cost of your included primary care office visits and membership benefits.

Additional costs may apply for services that are not included in your membership, such as:

• Laboratory testing

• Imaging (X-rays, CT scans, MRI, etc.)

• Medications and vaccines

• Procedures

• Specialized testing (such as DUTCH®, GI-MAP®, and food sensitivity testing)

• Home visits

• IV therapy and wellness injections (member discounts apply)

• Services provided by specialists, hospitals, or other outside facilities

We'll always discuss any recommended services and associated costs with you beforehand, so you'll know exactly what to expect—no surprise bills.

Yes! Telehealth visits are included as part of your membership and offer a convenient way to receive care without coming into the office.

Are home visits included with my membership?

Yes! Members receive one complimentary in-home visit upgrade each calendar year, allowing one eligible office visit to be provided in the comfort of your home at no additional cost.

Whether you're home with a newborn, feeling too sick to drive, recovering after surgery, or simply prefer the convenience of being seen at home, we're happy to bring your care to you. Additional home visits are available for an additional fee.

Why a Membership Model?

I built Root & Rise Family Health around a membership model because I believe healthcare should be accessible, affordable, and centered around the patient—not the limitations of a traditional healthcare system.

Too often, people delay seeking care because they're worried about copays, deductibles, surprise bills, or whether an appointment is "worth it." My goal is to remove as many of those barriers as possible so you can reach out when you need care—not just when you can justify the cost.

A membership model allows us to focus on what matters most: building a lasting relationship, spending the time needed to truly listen, and creating personalized care plans that address the root cause of your concerns rather than simply treating symptoms.

While insurance plays an important role in healthcare, it shouldn't determine how much time you receive with your provider or whether you feel comfortable asking questions. My goal is to make high-quality primary care more accessible, more personal, and more predictable, with transparent pricing and a provider who knows you.
